Statement of Ownership and Purpose

Services, Inc., a marketing research company located at 500 North Michigan
Avenue in Chicago, Illinois.
The purpose of the Icebox Advisors Panel is to gather opinions about food
products and services of many kinds from our members, and learn what they think
about many issues, in order to advise our clients on marketing issues. All
opinions expressed in our research projects are the property of C&R
Research Services, Inc. or the client sponsoring the research.

Your privacy is important to us. To better protect your privacy we provide this
notice explaining our online information practices and the choices you can make
about the way your information is collected and used. To make this notice easy
to find, we make it available on our homepage and at every point where
personally identifiable information may be requested.
We recognize that the good will of the individuals who agree to be interviewed
is the lifeblood of the survey research Industry, and we take it as our duty to
protect the identity of individual respondents as well as what is known as
"personally identifiable information" from disclosure to third parties -
including government agencies, sales organizations, the public or media -
unless we are legally compelled to do so.
The Icebox Advisors Panel engages in Marketing Research only
Icebox Advisors conducts marketing research projects of various kinds among its
panel members. We communicate with panel members about these projects and about
issues regarding the panel and its management only.
Icebox Advisors sells nothing to its panel members, and no member of the panel
will ever be asked to buy anything because of their participation. We do not
give, sell, lend or otherwise transfer or share your information with others,
except as described below (The Information We Collect and How It Is Used).
The marketing research projects we conduct are designed to collect information
about consumer opinions and behavior. Research topics may include, but are not
limited to, advertising, products, services, hobbies, interests, health, family
relationships, and life experiences. Our projects are not sales efforts, nor
will information offered in a research project ever be used in a direct sales
effort to those who participate in the survey.
Information from our projects will generally be used by our clients to develop
new products or services, improve existing products or services, and to develop
advertising or marketing programs.
The Types of Marketing Research Projects We Conduct
Icebox Advisors conducts many types of marketing research projects, including
but not limited to online (web) surveys, interactive discussions, bulletin
boards, focus group discussions, individual interviews, in-person interviews
and visits, in-home testing of products, and telephone interviews. Panel
members will receive invitations to participate in various projects and, of
course, their participation is entirely voluntary.
How we communicate with panel members
Communication between Icebox Advisors and its members is done primarily via
e-mail, using an e-mail address supplied during the sign-up procedure. However,
we may also communicate with panel members by mail or telephone.
Icebox Advisors online projects may be conducted using our online survey portal,
www.netcrimson.com, but we may occasionally conduct surveys at other sites
operated by one of our clients or partners. Any site or physical facility that
we use for a research projects will have agreed to conform to standards and
privacy policies.
The Information We Collect and How It Is Used
This notice applies to all information collected or submitted on the
IceboxAdvisors.com website, to survey data collected on our online survey
portal, www.netcrimson.com, and to information collected on any website
operated by C&R Research.
We collect three kinds of information: (1) the opinions of our panel members
about issues of interest to us or our clients, (2) background information about
our panel members and their households, and (3) personally identifiable
information about the members of the Icebox Advisors panel. The way this
information is used is as follows:
1. The opinions of our panel members about issues of interest to us or our
clients.
These questions may cover any number of areas, but generally will involve
products or services and advertising related to those products and services.
Examples of such products and services include food products, beverages,
websites and other Internet services, TV show, movies, music, sports, styles
and trends.
Opinions are generally reported in aggregate - 43% of survey respondents
thought this-or-that, or 23% of survey respondents over 30 were aware of
something-or-other. This information will be shared with our clients without
identifying the identity of any individual.
2. Background information
During the process of registering to become a member of the Icebox Advisors
panel new members will be asked a number of questions about themselves and
their households. Some of the types of background information that we collect
include, but are not limited to:
-
What was the highest level of education you completed?
-
What is your employment status?
This information is used as background information, so that we can determine
which members of our panel should be invited to the surveys we conduct, and to
better interpret survey findings. For instance , if we were to
conduct a survey about cable TV shows for people 25-54, we would need to invite
panel members whose households had cable service and members between the ages
of 25-54.
ackground information is combined with opinion data and reported in aggregate -
for instance, we might report that, "43% of all survey participants have
watched some television show, but among those with cable or satellite TV
service, this rose to 64%."
In the case of what is known as "open-ended responses"- for instance, a survey
question in which you are asked to explain why you feel a certain way - or an
opinion offered in a bulletin board, chat discussion, or focus group
discussion, we identify the person offering the opinion in an anonymous manner:
for instance, "a 35 year-old-man from Texas" or "a female purchaser of
Such-and-Such a product."
3. Personal information, and information that, although it may not itself
identify an individual, can be used to do so.
When someone joins the Icebox Advisors panel, we collect personal information
in order to communicate by mail, e-mail or telephone with them.
Icebox Advisors may share information with a fulfillment (mailing) house in
order to send gifts or prizes to our panel members, or to vendors or
subcontractors engaged in data management for us. A fulfillment house will be
given only the information necessary to perform their duties - for instance,
name and postal address information to use in a mailing - not answers to survey
questions or other similar data. Any fulfillment house or other vendor managing
a program for us will agree to be bound by the privacy rules and restrictions
contained in this notice.
The types of personal information that we collect may include, but are not
limited to:
-
First and last name
-
Members of household first and last name
-
Mailing address
-
E-mail addresses for all registered members
-
Phone number
We will never use or share the personally identifiable information provided to
us in ways unrelated to the ones described above unless required to do so by
subpoena, or without first obtaining explicit permission to do so, and
providing you an opportunity to opt out or otherwise prohibit such unrelated
uses.
Can members of the panel ever communicate directly with our client
companies?
Icebox Advisors does not provide this service. Most of our clients maintain
websites and 800 numbers, and publish mail or e-mail addresses for this
purpose, and these can be found in many public directories or using web search
tools, such as www.google.com.
Our Commitment To Data Security
To prevent unauthorized access, maintain data accuracy, and ensure the correct
use of information, we have put in place what we believe to be appropriate
physical, electronic, and managerial procedures to safeguard and secure the
information we collect online.
